JOB DESCRIPTION LICENSES VOCATIONAL NURSE

QUALIFICATION/EXPERIENCE:

• Must have a current Texas license for Nursing, with a clear record from the Texas Misconduct Registry.
• Must have at least one year of experience in geriatric nursing.
• Must have a current TB test and CPR card.
• Must be capable of lifting and transferring clients as necessary.

Responsible OF LVN:

1. Perform duties as assigned by the RN. Assist the RN as required.
2. Practice within the LVN Standards of practice for the state of Texas.
3. Provide care and assistance for each client as required, following their plan of care.
4. Administer medication/ treatments in a timely manner, documenting on the appropriate records including date, time, dose, and initials.
5. Observe and report side effects of medication to the client's doctor and family.
6. Council with clients that self-medicate ensuring the client is aware of the purpose of the medication, schedule and dose of medication, and potential side effects. Follow up the counseling with appropriate documentation on the Medication Self Administration Test.
7. Carry out dietary instructions provided by the Registered Dietitian Consultant for each client.
8. Document any incidental events as necessary in the Nurses Notes.
9. Obtain physician orders for all necessary medications/treatments. Complete and mail out verbal order forms to the appropriate office. File copies according to procedure.
10. Complete and document all weights ad vital signs monthly.
11. Initiate and complete Incident Reports in a timely manner. Report incident to RN, Executive Director, physician, and family as appropriate.
12. Test the security system to be sure it is operating on a daily basis ensuring the optimal health and safety of each client.
13. Perform all duties in a safe and efficient manner, observing universal precautions at all times.
14. Present a professional appearance and behavior, to families, staff and visitors.
15. Observe all HIPPA requirements for confidentiality at all times.
16. Provide copies of current CEU's to Executive Director as they are obtained.
17. Discuss nursing concerns with the RN; make recommendations for care as necessary.
